APPOINTMENT SCHEDULING
Please call our office and our staff will gladly assist you in scheduling your first or follow-up appointment. Please note that an office visit is required before we can schedule any procedure. We will call you to confirm your appointment 1 week prior and we ask that you kindly return any messages so that we can confirm or cancel all appointments. All appointments not confirmed by 8:00 am the day prior to your appointment will be removed from the schedule. Any confirmed appointment that needs to be canceled by the patient must be canceled by 8:00 am prior to the day of the appointment or a $75.00 fee will be charged.
INSURANCE REFERRALS & IN-NETWORK PROVIDER VERIFICATION
Prior to your appointment please check with your insurance company to see if a referral is required. If you need a referral you need to request it from your primary care physician. Please have the referral faxed to our office in advance or bring it with you to your visit. Failure to provide a referral may result in the need to reschedule your appointment which is subject to the late cancellation fee of $75.00.
It is your responsibility to verify that Dr. Crenshaw is an in-network provider. Please check with your insurance provider or call our office to confirm that we participate with your insurance.
